Start with the ground
Ground information is a shared reference, not a document for one discipline.
Treat uncertainty explicitly
A ground report records evidence from particular locations and methods. It does not remove every uncertainty along a construction route. Begin the planning discussion by distinguishing what has been observed from what has been inferred, and keep the limits visible to every discipline relying on the information.
Share the question
A change in observed conditions can affect foundations, temporary works and construction access in different ways. Describe the question once with a common location reference. Each responsible discipline can then assess its effect without working from an incomplete retelling.
Record the response
Connect the approved response to its evidence and the drawing revision used on site. That link matters when the next work front encounters a related condition. A clear record supports professional judgement; it is not a substitute for equipment- and location-specific assessment.
